Clients of Quarternote Counseling are invited and encouraged to use the "Patient Portal", provided by our Electronic Healthcare Record vendor Therapy Notes, for the purposes of completing paperwork and reviewing client notices. Clients will receive a "Welcome Email" to register for the portal after their first (intake) session has been scheduled. Clients will subsequently receive an email each time there is a new document added to their portal for completion or review. Clients may also use the portal to make payments on their account at any time.
Quarternote is not responsible for the security of a client's portal account. Portal accounts should only be accessed by the client themselves (or the client's parent/legal guardian if the client is a minor). Clients are responsible for the creation and maintenance of their portal account's password and security. Clients are strongly encouraged to keep their portal login credentials private and refrain from sharing the information with anybody in the interest of keeping their electronic Protected Health Information (ePHI) secure.
Clients who are concerned that their portal login credentials have been compromised may take any of the following actions:
Request that Quarternote send a password reset link to the current email address on the client's account
Request that Quarternote revoke portal access for the email address on the client's account (this option will revoke access to the portal account - access may be retrieved at the client's request, the client will need to again register for use of the portal, and all existing documents within the portal account will be available)
Request that Quarternote change the email address on the client's account (this option will require a new portal account to be created by the client and access to the previous account and all documents within that account will no longer be available)
