Quarternote Counselingparticipates with several insurance companies and offers self-pay options for those clients either without insurance or who are choosing to forego using their insurance.
Quarternote currently participates with the following insurance companies:

"Fee for Service" Medicaid
Molina Complete Care of Virginia (a Medicaid plan)
Anthem HealthKeepers Plus (a Medicaid plan)
Quarternote will accept and bill out of network (aka non-participating) insurance as a courtesy ONLY if the policy provides out of network benefits. Please check availability of out of network benefits on your policy. Clients using out of network insurance will receive a Good Faith Estimate and will be responsible for paying the difference between Quarternote's billed amount and the amount paid by insurance. Clients will also be required to pay $100 each session, due at the time of service, until an Explanation of Benefits (EOB) has been received from the insurance company indicating the amount they will pay for services. Clients without insurance coverage, or choosing to forego using their insurance coverage, are welcome to be seen on a self-pay basis. Quarternote's fees are as follows:
Intake session (90-minutes) - $180.00
60-minute session - $150.00
Group sessions - varies based on group
Quarternote's late cancellation/no-show fee is $60 per occurrence. When cancelling an appointment, notice must be provided directly to the client's therapist of the cancellation. Notice must be provided to the therapist no later than 7p.m. the day prior to the scheduled appointment. Failure to provide notice by 7p.m. AND/OR missing an appointment with no notice (defined as a "no-show") will result in the fee being applied to the client's account. This fee is not covered by insurance.
In some instances, when eligible, a sliding scale may be available that would provide a fee reduction to the client. Sliding scale eligibility is determined based on the Federal Government’s “poverty guideline” practices and is based on the household's pre-tax (gross) income. Information about the Federal Government's poverty guidelines may be found here. Quarternote reserves the right to limit the number of clients utilizing the sliding scale. Good Faith Estimate Notice Clients being seen using out-of-network insurance or as self-pay (both full fee and using the sliding scale) are required by law to be provided a "Good Faith Estimate" prior to receiving services. The Good Faith Estimate provides a breakdown of the expected cost of the services you will receive from Quarternote and provides an estimate of the total amount you may pay for a 1-year period of time. You may ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may choose to dispute the bill. Always be sure to save a copy or picture of your Good Faith Estimate.
