Please note that the forms below are for completion only when the first appointment with our practice has been scheduled. If you do not currently have an appointment scheduled, please use the contact form on the “Contact Us” page to request a phone call for screening and scheduling.
A portion of the registration paperwork is available below. There are some forms that are required to be completed in the office with your therapist. If desired, you may complete the paperwork provided below ahead of time by downloading the forms via the link and completing them electronically or manually (we ask that all forms be signed by hand, please). If you choose to complete any or all of these forms ahead of time please bring them with you to your first appointment at our office.
We realize this is a significant amount of paperwork, but by completing the forms provided ahead of time, you will get the most value from your session. If you are unsure of any information being requested on these forms, please leave it blank and ask the receptionist for guidance upon arriving at the office for your first session. If you prefer to complete the entirety of the paperwork in the office on the day of your first appointment, you are more than welcome to do so.
Please ensure that, if the client is a minor, the parent/legal guardian provides their name, relationship to the minor client and signature in the appropriate areas on each form.
If you choose to complete your paperwork ahead of time and bring it to the office, we highly encourage you to place the completed forms in an envelope or folder so as to protect your personal information from view or being misplaced while traveling.
